Where do I find the parental consent form ?

The parental consent form is located in Cruise Control under parent consent form. Login to cruise control here. Under the Parent Consent form section, click on the 'Minors travelling with a Responsible Adult' and download the form. Follow the instructions on the form and have it signed by a parent or legal guardian, and email it to us. Please be sure to also carry a copy of this form with you as this may be requested at check-in.
